Themed Set #1: Q3

Today's question is the third of seven in our first themed set!

After a disaster on one of the earliest Apollo missions, a company was contracted to analyze the audio captured on the flight. What was the disaster and why was this audio never released?

Here is the answer to yesterday’s question!

The answer was the Telstar I satellite that was launched on July 10, 1962. This failed very soon (but still remains in orbit) since the US Government did not inform the company creating the satellite of a nuclear test that had taken place a day before, leading to very high amounts of radiation in the atmosphere
